Position Purpose: 

This position is responsible for documenting and tracking enrollments in Health Choice Arizona (HCA) Disease Management Programs, which aim to improve the health of HCA’s members with Serious Mental Illness (SMI) and chronic health conditions. The Population Health Technician also spends approximately 15% of their work time taking minutes for HCA Committees. Position duties include the following:


  • Document all new Disease Management Program enrollments in HCA’s care management platform and tracking database
  • Update changes to Disease Management Program enrollments in HCA’s care management platform and tracking database
  • Document member mailings to SMI/T19 member in the care management platform
  • Regularly report updates to status of Disease Management Program documentation to supervisor and Population Care Leads
  • Participate in Disease Management Program documentation process improvement projects
  • Participate in weekly team meetings
  • Take minutes for committee meetings as assigned by Chief Clinical Officer and Integrated Health Care Development Officer
  • Serve as backup to the Integrated Clinical Coordinator for management of multiple care management referral inboxes
  • Prepare materials for Recover Wellness Team provider site visits

Education / Experience / Other Requirements


  • High school diploma required
  • Associates degree preferred

Years of Experience:

  • 1-3 years of experience in a health care or information technology setting

Specialized Knowledge:

  • Knowledge of all Microsoft Office applications, especially Excel, Access (or other database programs), PowerPoint and Word

Skills & Abilities:

  • Ability to document activities accurately and timely in an electronic medical record system
  • Ability to prioritize and effectively handle multiple projects
  • Ability to work effectively in a team model
  • Ability to maintain confidentiality, privacy and security of protected health information and proprietary information
  • Ability to communicate professionally in written and oral formats
  • Ability to coordinate and support committees, record minutes accurately and professionally
