Position Purpose: The Manager, HRA Program is responsible for the day to day operations of the HRA Program within the department. This position oversees the activities to ensure all members who require a Health Risk Assessment receive one in a timely manner and assists with coordination of care. This position interfaces with members, families, and physicians as well as health plan staff including Medical Director(s) and other Health Choice personnel as needed.


  • Develop and monitor the process for completion of the Initial and Annual Health Risk Assessment required to remain in compliance with contractual standards
  • Assign and monitor initial and annual HRAs to staff
  • Audit HRAs to ensure compliance
  • Communicate and educate the job expectations to the HRA Nurse Specialists
  • Ensure new employees are adequately trained and have proper orientation
  • Manage staff, timecards, scheduling, yearly reviews, performance guidance and counseling
  • Complete documentation in electronic medical record and claims systems
  • Plan, assign , and monitor the HRA process and reports results to the Director
  • Monitors calls and documentation done by the HRA Nurse Specialist for quality purposes
  • Meet with Supervisor on a regular basis to assess and discuss program operations
  • Maintain accurate records of enrolled HCG members for the SNP Care Management Reports
  • Ensures HRA’s are completed within the identified timeframe as outline in contractual standard

Education / Experience / Other Requirements


  • Associate’s degree or Bachelor’s degree from an accredited Nursing School required

Years of Experience:

  • 2+ years managed care experience preferred
  • 1+ years Medicaid/Medicare experience preferred
  • Prior supervisor of staff experience

Specialized Knowledge:

  • Previous Health Risk Assessment and/or care management experience preferred
  • Previous Medicaid/Medicare plan experience preferred
  • Proficiency in computer and technology utilization required, including, but not limited to: care management software, data-analytics software, Microsoft Office Products (Outlook, Excel, Power Point, Word, Visio)

  • Ability to manage multiple projects and prioritize adhere to deadlines/time frames
  • Complete tasks with little supervision
  • Ability to supervise and lead others
  • Ability to manage large workload
